Please Note:  Due to a very high number of referrals and lengthy waiting periods for appointments, we are currently not accepting pain management referrals until further notice.
PRIMARY CARE

The Access to Care Program will match uninsured clients with a healthcare provider who has agreed to become a primary medical home for them.  Providers agree to see clients at free or discounted rates; focusing on both preventive and acute care office visits.

SPECIALTY CARE

The primary care providers will contact Julie Sykes, our Specialist Referral Coordinator, when an Access to Care client needs a referral to a specialist. Julie will contact the patient with an appointment date.
There may be a wait time for the specialist available.  Access to Care may not have a provider available for all specialty areas.

Limitations:
Access to Care is not a form of insurance coverage. The program will directly link enrollees with health care services volunteered by local health care providers. Access to Care cannot help with all health care services in all circumstances.
  • Access to Care may not be able to help with all types of care and service
  • Access to Care is not health insurance
  • Access to Care cannot pay past medical expenses.

If you have questions regarding specialist referrals, please call Julie Sykes at 330-926-5766

PHARMACY ASSISTANCE

After seeing a primary care physician, you may have prescriptions that you need to obtain.  Access to Care has a Pharmacy Coordinator who can help you find the best way to access your medications.  The pharmacy coordinator can help you:

  • Fill out applications for free medications through Patient Assistance Programs through the pharmaceutical companies

  • Direct you to the $4 generic medications that are available at many pharmacies

  • Answer questions regarding use of Ohio’s Best Rx cards and Executive Med cards that you receive when you are enrolled

If you have any questions regarding medications, please call Taria Parks, Pharmacy Coordinator at 330-344-4082
